Congenstive Heart Failure Clinic Minimize
Parkersburg Cardiology Associates, Inc. and Camden Clark Medical Canter have developed a Comprehensive Congestive Heart Failure Program whose mission is "To optimize the quality of life for patients with congestive heart failure by providing personalized, compassionate care, through medical management and comprehensive education."  It is designed to help patients manage their condition and maintain a healthy lifestyle. It is comprised of a multidisciplinary team, including the primary care physician, cardiologist, specialized nursing care, dedicated transition coach and nutrition experts.

Our program offers both, inpatient and outpatient therapy to our patients suffering from heart disease.  Upon admission to CCMC, a patient with a primary diagnosis of heart failure will be consulted by a cardiologists and seen and followed by a dedicated transition coach.  The coach will assure protocols and education are initiated. He/she will assist the patient in managing and optimizing their care.  The coach assures the patient has the appropriate follow up in our Congestive Heart Failure Clinic at Parkersburg Cardiology Associates within three days after discharge and arranges for any other post discharge needs such as home health or rehab follow up.

What is CHF?

The patient will have access to our clinic based on a referral from their primary care physician, the hospital transition coach or self referral. The CHF clinic can be contacted at 304-424-4574.  The patient will be evaluated by a cardiologist and/or a nurse practitioner, certified in heart failure, during the first appointment and an individualized treatment and education plan will be created.  The patient will be scheduled regular appointments in the clinic based on their condition.  They will also have regular appointments with their primary care physician.

With the right attention and treatment, heart failure will not stop a patient from doing the things they enjoy.

Our CHF Clinic Minimize
Parkersburg Cardiology Associates and Camden Clark Medical Center’s CHF clinic is an education and treatment program and is designed to control the patient’s disease and maximize their quality of life. The patient and their physician are active participants in their treatment plan.  We offer testing capabilities that include, EKG’s echocardiograms, stress testing, lab testing and external counterpulsation.  The staff will work very closely with the attending physician to keep them fully apprised of the patient’s progress.  In the clinic, the patient will learn medication compliance, making dietary changes as needed and make lifestyle changes such as monitoring their weight on a daily basis.  The patient will also see their primary care physician regularly so their doctor can make sure they are staying healthy as well.

Symptoms Minimize
The best known symptom is shortness of breath.  Heart failure may result from excess fluid in the lungs, making it difficult to breath.  Being tired is often another common symptom.  As the heart works harder to pump properly, your muscles and other tissues receive less oxygen and nutrition.  Without proper fuel, the body cannot perform as usual, and is tired.  Swelling of the feet, ankles, legs and occasionally, the abdomen are other symptoms of Congestive Heart Failure. Many times, this is accompanied with sudden weight gain.
